NW Anxiety Fees

All services are billed at the following rates:

In-Office Rates*

Senior Clinician
$250 - Intake Diagnostic Evaluation
​$225 - Follow-up Therapy Session

Associate Clinician
$250 - Intake Diagnostic Evaluation
​$175 - Follow-up Therapy Session

Training Clinician
$150  Intake Diagnostic Evaluation
​$65 - $150 Follow-up Therapy Session​​

In-Home/Community-Based Rates*

$350 - Intake Diagnostic Evaluation

Fees are due upon initiation of session. Refunds are not available. If you’re unable to make an appointment, 24-hour notice is required or the full fee charges are assessed.
*Insurance Disclosure
​NW Anxiety Institute is not in-network for any insurance companies.
​However, you may consult with your insurance company to determine reimbursement eligibility.
Reimbursement is considered a matter between you and your insurance company. 

​For individuals insured by OHP (Medicaid) or Kaiser Permanente for mental health care, NWAI services might be covered by your insurance. For more information, contact your insurance about the steps necessary to receive a referral (or authorization) for NWAI services. 

Sliding Scale

For individuals interested in low-cost treatment provided by graduate student interns, we are happy to add you to our waitlist for discounted services. As a teaching institution, we strive to provide our interns with the highest level of training possible, ensuring quality care for all patients. Contact us for more information.​

Insurance Information

Do you take my insurance?
The NW Anxiety Institute is a contracted provider with Kaiser Permanente and requires an internal referral. We are also credentialed with the Oregon Health Plan (OHP/Medicaid). We are not in-network for any other insurance companies.

Can I still get reimbursed by my insurance company?
Most insurance plans do provide the opportunity to apply for reimbursement for therapy services received from out-of-network providers. In instances of insurance reimbursement, we ask each individual to pay for services and accept reimbursement from the insurance. You will be provided with a bi-monthly billing statement for the services rendered and fees paid to submit to your insurance. Please call your insurance company before your admission and ask the following questions:

  • Does my insurance plan have out-of-network mental health benefits?
  • What is my annual deductible and has it been met for this year?
  • When does my calendar year start?
  • How many sessions per the calendar year does my plan cover?
  • Is an authorization required? If yes, how do I obtain it?
  • What percent of treatment is reimbursed per visit for an out-of-network provider?
  • What is the dollar amount recognized per visit (usual and customary rate or UCR)?
  • What forms do I need to submit for reimbursement? (Note: most insurance companies require that you submit a Monthly Billing Statement and a Treatment Plan from your therapist together with a standard HCFA 1500 claim form completed by you. Find out if there are any other forms required and how to obtain them).
  • What address should I mail my claims to?
  • Are there any time limitations on filing my claims for reimbursement?
  • If interested in the Focus Program: Am I eligible for reimbursement for out-of network intensive outpatient services provided by the NW Anxiety Institute Intensive Outpatient Program NPI# 1043649080?
